Palliative Care Connect provides information and links to palliative care and bereavement supports for people with life-limiting illnesses and their family, friends, and carers, as well as health and aged care professionals.
Palliative Care Navigation helps people experience seamless, joined up care and support.
Palliative Care Connect has been funded through a $7.27 million grant from the Australian Government. The goal of the service is to increase access to palliative care and bereavement supports by assisting with smoother transitions between types of care and providing guidance into end-of-life care pathways.
Statewide Palliative Care Navigation and Support
Palliative Care Navigators, available through the phone line, provide palliative care information, service linkages, and empower individuals to make choices aligned with their culture and preferences.
Palliative Care Navigators also support general practitioners and other healthcare professions, including those who work in palliative care units, acute care and aged care services to organise and link people to appropriate palliative care, supports or services.

Aboriginal Specific Navigation
Aboriginal people can contact Palliative Care Navigators to find out how they can be supported in a culturally respectful manner. Health, aged care, disability, and community service professionals supporting an Aboriginal person can also contact the service for advice. Telephone support available statewide and face-to-face in Port Augusta, Whyalla, Port Lincoln and Ceduna.

Statewide Bereavement Navigation and Support
- Bereavement Navigators, available through the phone line, provide personalised support for people of all ages preparing for or adjusting to the death of someone, regardless of the circumstances. Face-to-face services are also available in Adelaide, Limestone Coast, and the Riverland.
- Bereavement counselling and groups are offered by grief-qualified professionals.
- Grief workshops, available online or in person, help organisations and community groups strengthen their support for those grieving.
